1. Schedule Your Telehealth Consult

After completion of the form below, one of our Patient Care Concierge Team Members will contact you to schedule your Telehealth consult with your provider.

You will have an hour with your provider to discuss your medical history, health risks, goals, and next steps in your journey to wellness.

2. Gather Your Information

Before your appointment, our Patient Care Concierge will send you the new patient paperwork to fill out. This form gathers information about your health history, family health history, eating habits, sleep habits, and more. This will be reviewed by your provider prior to your virtual visit.

3. Follow the Link to Wellness

Our Patient Care Concierge will send an access link to be used at the time of your appointment. This is a secure, HIPPA compliant video platform.

4. Receive Your Plan and Get Started!

Your Telehealth visit includes: The elements above all work to create your Individualized Treatment Plan – Your POWER RX®.

5. Treatment Plan Review

Our Patient Care Concierge team will call to review your treatment plan and talk through your providers treatment and supplement recommendations, schedule follow-up appointments, health coaching and/or nutrition appointments and any additionally recommended services.

6. Follow Up

Your Patient Care Concierge will reach out to you to schedule your follow-up Telehealth consult. Follow-ups are typically scheduled 3-4 weeks from your initial appointment, and will depend on if additional labs were requested after your initial consult.

Specialty Labs

Some of our integrative labs require a special kit, and we offer complimentary drop shipping of this lab kit directly to your door should they be requested after your initial virtual consult.

These specialty labs include genome testing for weight management, food allergies and intolerances, nutritional deficiencies, saliva hormone testing, toxin testing including mold, heavy metals and glyphosate, lyme testing, and PANS/PANDAS testing.
